HIV/AIDS Program Sustainability Analysis Tool (HAPSAT)
The HIV/AIDS Program Sustainability Analysis Tool (HAPSAT), was developed by Health Systems 20/20, to assist governments and donors with the development of HIV/AIDS policies and implementation plans. HAPSAT utilizes a computer-based model for forecasting and analyzing the sustainability of HIV/AIDS programs during periods of service delivery scale-up and/or when facing limited or unknown future resource levels.

HAPSAT produces estimates of the financial and human resources required to sustain and/or scale up a comprehensive portfolio of quality HIV/AIDS services. A comprehensive sustainability analysis is possible with HAPSAT because it covers the core areas of HIV/AIDS resourcing.

Various parts of the HAPSAT analysis have been customized to serve country needs. In Zambia, it was used to inform an application to the Global Fund, in Nigeria it supported a World Bank MAP application and in Côte d'Ivoire it informed the development of USAID's country operational plan. It is currently being implemented in Ethiopia, Kenya, Haiti, and the Democratic Republic of Congo.

The HAPSAT tool enables policy and implementation officials to estimate the amount of financial and human resources necessary for the successful delivery of services within a national HIV/AIDS program. Given information about expected resource availability and the current and target levels of service delivery over the short to medium term, the HAPSAT analysis, will identify expected gaps. Moreover, the HAPSAT tool can be used to simulate proposed solutions for addressing resource gaps or service delivery gaps. Thus HAPSAT is useful for generating evidence regarding the potential of specific policies to contribute to the attainment of established goals.

HAPSAT is a tool that allows strategic information from multiple sources to be synthesized and leveraged to provide transparent, flexible evidence-based decision-making.

A sustainable national HIV/AIDS program is one that delivers the planned set of services across programmatic areas, at a desired level of quality across the plan’s timeframe. Such a program operates within the available resource envelope without jeopardizing other non-HIV/AIDS health services, and with minimal risk of sharp decreases in the quantity or quality of services provided to patients over the medium term.
- Do I need to pay for HAPSAT access?
The HAPSAT’s development by Health Systems 20/20 is funded by the United States Agency for International Development (USAID); it is free and available for download on this website. Health Systems 20/20 can provide technical assistance with implementation of HAPSAT into an M&E or strategic planning process. Contact your country USAID office for further information regarding technical assistance from Health Systems 20/20. We encourage anyone planning to use the tool to inform policymaking to contact us.
- What kind of computer software will I need to use HAPSAT?
To utilize the HAPSAT tool, you will need to have Excel 2003 or a later version that accepts the Excel 2003 platform.

To learn how to use the HAPSAT model, we have multiple learning tools for your convenience. To begin, we encourage you to download the Introductory Guide which will walk you through each step of using the model and illustrate the use of the HAPSAT tool in each step including model specification, scenario specification, analysis and reporting. - What data are needed to run the HAPSAT and where can I obtain it?
The HAPSAT model utilizes a wide range of primary data to project the costs and resource needs for each program area. To enable you to conduct this aspect of the activity, we would encourage you to read our data collection guide, available for download on this website along with sample surveys which can be adapted to your country’s context.
The HAPSAT model can be harmonized with the SPECTRUM policy modeling system to make demographic and epidemiological projections. The parameters required for these projections may also be obtained from open online sources such as the UNAIDS website.
